I hereby grant permission to Ballston Area Community Center and its representatives to photograph, film, and/or record me (or my minor child) during participation in activities and events associated with the teen program. I understand that these images and recordings may be used in promotional materials, including but not limited to websites, social media, brochures, flyers, and other media platforms.

I acknowledge that these images and recordings may be used without further notification or compensation and that all rights to these materials belong to Ballston Area Community Center. I also understand that I may revoke this consent at any time by providing written notice to Ballston Area Community Center, but that such revocation will not affect prior uses of my likeness in previously published materials.
I release and discharge Ballston Area Community Center, its employees, agents, and affiliates from any claims, demands, or causes of action that may arise in connection with the use of these images or recordings.
